Background Turf toe injuries, though most common in athletes, can also occur in non-athletes. No study exists in the current literature investigating operative outcomes in non-athlete patients with chronic turf toe injury. In this study, we present our outcomes on operatively treated turf toe injuries in non-athletes in the only cohort yet studied. Methods Using ICD-10 codes, we assembled a cohort of 12 patients who underwent operative repair of chronic turf toe injury from January 2012 through January 2018 at the investigating institution. These 12 patients were evaluated to determine demographic information, method of injury, length of time from injury to surgery, clinical and radiologic characteristics of the injury, and operative outcomes including mean preoperative and postoperative VAS (Visual Analog Scale) scores, preoperative and postoperative FFI (Foot Function Index) scores, and postoperative complications. Results On initial clinical presentation, all 12 patients had local tenderness with associated painful range of motion. Four patients had restricted range of motion, all patients had a positive Lachman test, two had local edema, and eight had hallux valgus deformity. Mean VAS improved from 4.6 (range 2-9) to 1 (range 0-4). Mean FFI improved from 102.5 (range 56-177) to 61.75 (range 23-144). All patients had a negative Lachman test at final follow-up. No patients developed major complications or required revision surgery. Conclusions Our study is the first to investigate operative outcomes following chronic turf toe injury in non-athlete patients. Based on our study, surgeons and patients can expect significant improvement in overall pain and function following surgery.

Near-infrared (NIR) fluorescence imaging has been proved as an effective modality in identifying the tumor border and distinguishing the tumor cells from healthy tissue during the oncological surgery. Developing NIR fluorescent probes with high tumor to background (T/B) signal is essential for the complete debulking of the tumor, which will prolong the survival rate of tumor patients. However, the nonspecific binding and always-on properties of the conventional fluorescent probes leads to high background signals and poor specificity. Method: To address this problem, glutathione (GSH)-responsive, two disulfide-bonded dicyanine dyes (ss-diCy5 and ss-diNH800CW) were synthesized. As synthesized dyes are quenched under normal physiological conditions, however, once reached to the tumor site, these dyes are capable of emitting strong fluorescence signals primarily because of the cleavage of the disulfide bond in the tumor microenvironment with high GSH concentration. Besides, the GSH-responsive behavior of these dyes was monitored using the UV-vis and fluorescence spectroscopy. The diagnostic accuracy of the aforementioned dyes was also tested both in tumor cells and 4T1-bearing mice. Results: The fluorescence signal intensity of disulfide dicyanine dyes was quenched up to 89% compared to the mono cyanine dyes, thus providing a very low fluorescence background. However, when the disulfide dicyanine dye reaches the tumor site, the dicyanine is cleaved by GSH into two mono-dyes with high fluorescence strength, thus producing strong fluorescent signals upon excitation. The fluorescent signal of the dicyanine was enhanced by up to 27-fold after interacting with the GSH solution. In vivo xenografts tumor studies further revealed that the fluorescence signals of aforementioned dyes can be quickly recovered in the solid tumor. Conclusion: In summary, the disulfide dicyanines dyes can provide a promising platform for specific tumor-activatable fluorescence imaging with improved T/B value.

This communication reports novel luminescent rhenium(I)-polypyridine complexes appended with a perylene diimide (PDI) or benzoperylene monoimide (BPMI) moiety through a non-conjugated linker. Polymer based core cross-linked nanoparticles (CCNPs) have generated a lot of interest as potential stimuli-responsive drug delivery systems. In the present work, we have synthesized smart redox-responsive water soluble polymeric CCNPs by cross-linking water-soluble PEG based copolymers with bis(acryloyl)cystamine via isoxazoline bond formation through a 1,3-dipolar cycloaddition reaction (click reaction) without using a copper catalyst, in a water-THF mixed solvent. The successful synthesis of CCNPs was confirmed by NMR, GPC and FT-IR measurements. Size distribution of the precursor copolymers and the CCNPs was determined by DLS measurement. AFM and FESEM images have confirmed globular morphology of these CCNPs. Their high stability in a physiological environment makes them effective as potent drug carriers with high loading capacity. MTT assays confirmed the biocompatibility of the synthesized CCNPs. Favourable cellular internalization of these DOX loaded CCNPs into cancer cells and redox-responsive release of DOX therefrom make these CCNP potentially smart vehicles to deliver anticancer drugs into cancer cells.

A new kind of redox responsive nanoparticles was prepared through the modification and crosslinking reaction of xanthan gum. The nanoparticles had regular spherical shapes with diameters ranging from 110 to 180 nm. The nanoparticles showed good redox responsiveness. Doxorubicin (DOX) was loaded in the nanoparticles via ionic interaction with SO3-, for increasing the drug loading rate and avoiding drug leakage. The in vitro drug release could be controlled by pH and reduction conditions simulated to the internal environment of tumor cells. The nanoparticles were biocompatible and could be potentially applied as anti-cancer drug vehicles for targeted release.

A triply responsive amphiphilic polymer containing poly (ethylene glycol) (PEG) and pyrene linked by a disulfide bond (PEO-SS-PY) was synthesized. Benefiting from its amphiphilicity, PEO-SS-PY could self-assemble into 126 nm micelles in water. PEO-SS-PY with CB[7] forms a 1:1 pseudorotaxane polymer (PEO-SS-Py@CB[7]) in aqueous solution. After the formation of host-guest complex, the micelles size increased to 136 nm and the fluorescence emission intensity increased. Glutathione would break the disulfide bond, leading to the dissociation of the micelles. Spermine and adamantanamine hydrochloride would dissociate the host-guest complex, leading to change of micelle size and decrease of fluorescence intensity. More interestingly, this self-assembly micelle displayed highly sensitive response to Fe3+. Because the effective absorption of Fe3+ hindered the absorption of PEO-SS-Py@CB[7] at the excitation wavelength of 405 nm, Fe3+ distinctly quenched the fluorescence emission of PEO-SS-Py@CB[7], whereas, a very low changes of intensity was observed for other metal ions. Results of this paper provide a new idea for the combination of metal ion detection and controlled release.

Triptolide (TP) is one of the most common systemic treatments for inflammatory and immune diseases in China for centuries. However, TP exhibits some disadvantages, such as poor solubility in water, poor bioavailability, liver toxicity, renal toxicity, and other side effects. In order to reduce the adverse effects of TP, researchers have developed numerous strategies to address the adverse properties of triptolide. Nano-carrier-based triptolide delivery systems represent an emerging technology and are one of the strategies of nanomedicine that combines diagnostic and therapeutic applications in a single agent. In this approach, we developed a glutathione-activated carrier-free nanodrug of triptolide (CyssTPN) as a trackable drug delivery system. In this system, CyssTP self-assemble to form a carrier-free nanodrug, which possesses a monodisperse spherical morphology with hydrodynamic average sizes of about 50 nm. In addition, CyssTPN had good stability under different physiological conditions (pH, high salt, etc.). Apart from cellular imaging and cell uptake, CyssTPN can be tracked by the activation of TP ability in real-time and applied for cancer cell treatment efficiently. The result showed that CyssTPN could improve solubility, reduce the side effects, and increase the bioavailability of triptolide. It could also track triptolide activation timely and tumor therapy successfully.

Background: A preoperative marker for morbidity in patients with colorectal cancer would help to risk stratify patients and allow for timely intervention to avert poor outcomes. We conducted this study to evaluate preoperative lymphocyte-white blood cell ratio (LWR) as a marker of postoperative morbidity. Methods: A prospective cohort of patients who underwent elective surgery for colorectal cancer was reviewed. Three morbidity-related outcomes were described-overall morbidity, multiple morbidities, and severe morbidity, defined as Clavien-Dindo Class >= 3. Univariable and multivariable analyses of presurgical predictors of these three outcomes were performed. Preoperative variables included hemoglobin levels, neoadjuvant therapy, albumin levels, white blood cell count, lymphocyte count, LWR, neutrophil-lymphocyte ratio, and prognostic nutritional index. Results: Of 177 patients, 31.6% (56/177) suffered at least one morbidity, 15.3% (27/177) had multiple morbidities, 7.9% (14/177) suffered severe morbidity. On multivariate analysis, only LWR <0.180 (odds ratio [OR] 2.53, 95% confidence interval [CI] 1.15-5.55) and neoadjuvant therapy (OR 2.49, 95% CI 1.16-5.24) were associated with overall morbidity. For multiple morbidities and severe morbidity, only LWR <0.180 was significantly associated on multivariate analysis with an OR of 2.92 (95% CI 1.19-7.13) and 4.62 (95% CI 1.45-14.73), respectively. Conclusions: LWR is a preoperative marker which can be conveniently applied using standard preoperative blood tests.
