Background: Recent data have suggested a substantial incidence of atrial arrhythmias (AAs) in cardiac sarcoidosis (CS). Our study aims were to first assess how often AAs are the presenting feature of previously undiagnosed CS. Second, we used prospective follow-up data from implanted devices to investigate AA incidence, burden, predictors, and response to immunosuppression. Methods and Results: This project is a substudy of the CHASM-CS (Cardiac Sarcoidosis Multicenter Prospective Cohort Study; NCT01477359). Inclusion criteria were presentation with clinically manifest cardiac sarcoidosis, treatment-naive status, and implanted with a device that reported accurate AA burden. Data were collected at each device interrogation visit for all patients and all potential episodes of AA were adjudicated. For each intervisit period, the total AA burden was obtained. A total of 33 patients met the inclusion criteria (aged 56.1 +/- 7.7 years, 45.5% women). Only 1 patient had important AAs as a part of the initial CS presentation. During a median follow-up of 49.1 months, 11 of 33 patients (33.3%) had device-detected AAs, and only 2 (6.1%) had a clinically significant AA burden. Both patients had reduced burden after CS was successfully treated and there was no residual fluorodeoxyglucose uptake on positron emission tomography scan. Conclusions: First, we found that AAs are a rare presenting feature of clinically manifest cardiac sarcoidosis. Second, AAs occurred in a minority of patients at follow-up; the burden was very low in most patients. Poly(ethylene glycol) (PEG) diacrylate copolymer networks containing pentafluorophenyl active esters can be quantitatively substituted with a wide variety of primary amines, enabling the development of a versatile synthetic platform for the preparation of polymer hydrogel and membrane libraries. By tuning the starting network, a high degree of control over cross-linking density, water uptake, and functional group incorporation can be reproducibly achieved, which is vital for elucidating structure-property relationships in ion transporting membranes. From the same cross-linked scaffold, a diversity of basic, acidic, and solute-chelating moieties were obtained through functionalization, which allows for tailored uptake of basic and acidic organic dyes and metal chloride salts. Ion permeation and sorption measurements for a series of polymer networks with controlled cross-linking density and varied imidazole grafting densities illustrate the ability of this platform to isolate the effect of chemical functionality on ion transport from the effects of cross-linking density and water content.

Background Turf toe injuries, though most common in athletes, can also occur in non-athletes. No study exists in the current literature investigating operative outcomes in non-athlete patients with chronic turf toe injury. In this study, we present our outcomes on operatively treated turf toe injuries in non-athletes in the only cohort yet studied. Methods Using ICD-10 codes, we assembled a cohort of 12 patients who underwent operative repair of chronic turf toe injury from January 2012 through January 2018 at the investigating institution. These 12 patients were evaluated to determine demographic information, method of injury, length of time from injury to surgery, clinical and radiologic characteristics of the injury, and operative outcomes including mean preoperative and postoperative VAS (Visual Analog Scale) scores, preoperative and postoperative FFI (Foot Function Index) scores, and postoperative complications. Results On initial clinical presentation, all 12 patients had local tenderness with associated painful range of motion. Four patients had restricted range of motion, all patients had a positive Lachman test, two had local edema, and eight had hallux valgus deformity. Mean VAS improved from 4.6 (range 2-9) to 1 (range 0-4). Mean FFI improved from 102.5 (range 56-177) to 61.75 (range 23-144). All patients had a negative Lachman test at final follow-up. No patients developed major complications or required revision surgery. Conclusions Our study is the first to investigate operative outcomes following chronic turf toe injury in non-athlete patients. Based on our study, surgeons and patients can expect significant improvement in overall pain and function following surgery.

In this paper, a novel redox-responsive nanoparticles has been designed for targeted delivery of docetaxel (DTX). Chondroitin sulfate (CS) was used to construct the nanoparticles due to the ability of tumor targeting through binding with CD44 receptor that overexpresses on the surfaces of various tumor cells. A redox-responsive small-molecular DTX prodrug was prepared through modifying with cystamine containing disulfide bonds (Cys-DTX). Then the DTX prodrug was grafted to the CS to construct the amphiphilic polymer (CS-ss-DTX). Further, Cys-DTX/CS-ss-DTX nanoparticles were formed by self-assembly of amphiphilic polymer and incorporation of free Cys-DTX prodrug. This category of nanosized DTX delivery system was expected for not only exhibiting high permeability and cytotoxicity of Cys-DTX prodrug, but also targeting transportation of encapsulated redox-responsive Cys-DTX prodrug. According to results of related researches on physicochemical properties and biological evaluation, the novel redox-responsive Cys-DTX/CS-ss-DTX nanoparticles increased amount of DTX released from the nanoparticles in reductive environment, improved permeability in tumor tissues, enhanced cytotoxicity and decreased side effects compared with free DTX. All of these results showed that this kind of Cys-DTX/CS-ss-DTX nanoparticles were worthy of being expectation in tumor chemotherapy in future.

Chemo-photodynamic combined therapy is promising in cancer treatment, although low tissue penetration of visible light for activating photosensitizers (e.g., chlorin e6, Ce6) limited its broad applications. Combination of upcoverting nanoparticles (UCNPs) with the photosensitizers endows us with the possibility to utilize highly tissue penetrable near-infrared light; nevertheless, the mismatch between absorption of common photosensitizers (lambda(abs), mainly red) and emission of UCNPs (lambda(em), mainly green) resulted in low energy utilization and unsatisfied therapeutic efficacy in the current UCNP-PDT (photodymanic therapy) platforms. To resolve this problem, herein, we construct polymer-UCNP hybrid micelles (PUHMs) for codelivery of doxorubicin (DOX) and Ce6, and systemically studied the effects of spectral match between lambda(em) of UCNPs and lambda(abs) of Ce6 on efficiency of synergistic chemo-photodynamic therapy. Compared with spectrally mismatched PUHMs, the spectrally matched PUHMs can significantly enhance the utilization efficiency of upconverted emission energy to activate the photosensitizers and generate more reactive oxygen species (ROS) for enhanced photodynamic therapy. Meanwhile, as the assembled structure of PUHMs can be destroyed by the oxidation of ROS upon 980 nm laser irradiation because of the hydrophobic-hydrophilic transformation of poly(propylene sulfide) (PPS) segment, the spectrally matched PUHMs triggered faster release of DOX, thus resulting in more effective chemotherapy. As a result, the spectrally matched PUHMs induced more prominent cytotoxicity and superior synergistic therapeutic effect for cancer cells in vitro. Our results demonstrated that such spectrally matched PUHMs provide us with an effective strategy for photodynamic-chemo synergistic therapy.

Drugs, including small molecule anticancer drugs, have to be delivered and released into the cytoplasm or nucleus in order to elicit a therapeutic effect. In the present study, a novel corona core-shell nanoparticle was proposed for enhanced cellular uptake and light-responsive intracellular release. Light-responsive core-shell nanoparticles, i.e., gold nanoparticles-coated liposomes (CS), were prepared and further modified with hyaluronic acid (HA) (CCS). HA modification enhances the endocytosis of the nanoparticles by HA receptor-expressing cells, while the plasmonic properties of gold nanoparticles enable their light-triggered drug release. The results demonstrated that the uptake of CCS in HA receptor-expressing MDA-MB-231R cells was significantly enhanced compared with unmodified CS. Nanosecond pulsed laser irradiation (700 nm; 100 mJ/cm(2); 5 pulses) rapidly triggered the intracellular release of a fluorescent dye, 6-carboxyfluorescein (6-CF), from CCS and CS trapped in endolysosomes. The released 6-CF was evenly distributed throughout the entire cytosol and nucleus. Finally, the cytotoxicity of doxorubicin towards MDA-MB-231 cells was significantly increased by CCS delivery upon pulsed laser irradiation. In conclusion, with enhanced cellular uptake and light-triggered intracellular release, CCS significantly enhanced the therapeutic effects of doxorubicin, and may serve as a promising avenue for intracellular drug delivery.

Volumetric muscle loss (VML) injuries characterized by critical loss of skeletal muscle tissues result in severe functional impairment. Current treatments involving use of muscle grafts are limited by tissue availability and donor site morbidity. In this study, we designed and synthesized an implantable glycosaminoglycan-based hydrogel system consisting of thiolated hyaluronic acid (HA) and thiolated chondroitin sulfate (CS) crosslinked with poly(ethylene glycol) diacrylate to promote skeletal muscle regeneration of VML injuries in mice. The HA-CS hydrogels were optimized with suitable biophysical properties by fine-tuning degree of thiol group substitution to support C2C12 myoblast proliferation, myogenic differentiation and expression of myogenic markers MyoD, MyoG and MYH8. Furthermore, in vivo studies using a murine quadriceps VML model demonstrated that the HA-CS hydrogels supported integration of implants with the surrounding host tissue and facilitated migration of Pax7(+) satellite cells, de novo myofiber formation, angiogenesis, and innervation with minimized scar tissue formation during 4-week implantation. The hydrogel-treated and autograft-treated mice showed similar functional improvements in treadmill performance as early as 1-week post-implantation compared to the untreated groups. Taken together, our results demonstrate the promise of HA-CS hydrogels as regenerative engineering matrices to accelerate healing of skeletal muscle injuries.

Sequence specific molecules with high folding ability (i.e., foldamers) can be used to precisely control the distribution and projection of side chains in space and have recently been introduced as tailored systems for delivering nucleic acids into cells. Designed oligourea sequences with an amphipathic distribution of Arg- and His-type residues were shown to form tight complexes with plasmid DNA, and to effectively promote the release of DNA from the endosomes. Herein, we report the synthesis of new cell-penetrating foldamer sequences in which the foldamer segment is conjugated via a reducible disulfide bond to a ligand that binds cell-surface expressed nucleoproteins with the idea that this system could facilitate both assemblies with nucleic acids and cell entry. This new system was evaluated for delivery of DNA in several cell lines and was found to compare favorably with all comparators tested (DOTAP and b-PEI as well as a number of known cell penetrating peptides) in various cell lines and particularly in culture medium containing up to 50% of serum. These results suggest that this dual molecular platform which is long lasting and noncytotoxic could be of practical use for in vivo applications.
