Objective: To determine the pattern of presentations and characteristics of patients at a psychiatric emergency services facility. Methods: The cross-sectional study was conducted in November 2016 at the Department of Psychiatry and Behavioural Sciences, Jinnah Postgraduate Medical Centre, Karachi, and comprised all patients presenting to the departmental emergency services. Data was documented on a pre-designed semi-structured proforma, and analysed using SPSS 22. Results: Of the 316 cases, 162(51.3%) were males and 154(48.7%) were females. The overall mean age was 30.78+/-13.09 years (range: 8-80 years). Psychiatric emergencies comprised suicidal attempt, excitement, violence, altered sensorium, altered/inappropriate behaviour and extrapyramidal symptoms / acute dystonia together constituting for 123(38.9%) of the total presentations, while there were 20(6.33%) cases of hysterical fits and 6(1.89%) with acute anxiety symptoms. Overall, 56(17.7%) subjects were found to have no psychiatric diagnosis, and 46(81.5%) of these were referred cases. Conclusion: Non-emergency visits constituted high percentage of emergency presentation.

In this work, two novel boronate affinity monolithic materials able to extract glycopeptides within a polypropylene syringe are described and compared. The first material was synthesized from glycidyl methacrylate (GMA)-based monoliths modified with poly-3-mercaptopropyl-methylsiloxane (PMPMS) followed by attachment of 4-vinylphenylboronic acid (VPBA) via thiol-ene click reaction. The second material was prepared by using gold nanoparticle (AuNP)-modified monoliths as substrate followed by subsequent attachment of PMPMS and VPBA. The resulting materials were used as sorbents for solid-phase extraction (SPE) to selectively preconcentrate glycopeptides from horseradish peroxidase (HRP) digests. The material that gave the superior performance was that prepared with AuNPs due to the presence of abundant boronic acid groups, being its practical applicability also examined. The hybrid material exhibited a satisfactory efficiency of glycopeptide enrichment (identifying 24 glycopeptides from a total of 27) in mixture of tryptic digests of HRP and bovine serum albumin (BSA) (1:100, w/w). The sorbent shows low sensitivity (0.5 fmol/?L), good adsorption capacity (25 mg g-1) and suitable reusability (over 10 times). Moreover, the hybrid monolith was successfully applied to the selective enrichment of glycopeptides from human serum digests, without any pretreatment, in which 85 glycopeptides were identified by nano-LC-MS/MS, suggesting a great potential for application in glycoproteome field.

Volumetric muscle loss (VML) injuries characterized by critical loss of skeletal muscle tissues result in severe functional impairment. Current treatments involving use of muscle grafts are limited by tissue availability and donor site morbidity. In this study, we designed and synthesized an implantable glycosaminoglycan-based hydrogel system consisting of thiolated hyaluronic acid (HA) and thiolated chondroitin sulfate (CS) crosslinked with poly(ethylene glycol) diacrylate to promote skeletal muscle regeneration of VML injuries in mice. The HA-CS hydrogels were optimized with suitable biophysical properties by fine-tuning degree of thiol group substitution to support C2C12 myoblast proliferation, myogenic differentiation and expression of myogenic markers MyoD, MyoG and MYH8. Furthermore, in vivo studies using a murine quadriceps VML model demonstrated that the HA-CS hydrogels supported integration of implants with the surrounding host tissue and facilitated migration of Pax7(+) satellite cells, de novo myofiber formation, angiogenesis, and innervation with minimized scar tissue formation during 4-week implantation. The hydrogel-treated and autograft-treated mice showed similar functional improvements in treadmill performance as early as 1-week post-implantation compared to the untreated groups. Taken together, our results demonstrate the promise of HA-CS hydrogels as regenerative engineering matrices to accelerate healing of skeletal muscle injuries.

Background: A preoperative marker for morbidity in patients with colorectal cancer would help to risk stratify patients and allow for timely intervention to avert poor outcomes. We conducted this study to evaluate preoperative lymphocyte-white blood cell ratio (LWR) as a marker of postoperative morbidity. Methods: A prospective cohort of patients who underwent elective surgery for colorectal cancer was reviewed. Three morbidity-related outcomes were described-overall morbidity, multiple morbidities, and severe morbidity, defined as Clavien-Dindo Class >= 3. Univariable and multivariable analyses of presurgical predictors of these three outcomes were performed. Preoperative variables included hemoglobin levels, neoadjuvant therapy, albumin levels, white blood cell count, lymphocyte count, LWR, neutrophil-lymphocyte ratio, and prognostic nutritional index. Results: Of 177 patients, 31.6% (56/177) suffered at least one morbidity, 15.3% (27/177) had multiple morbidities, 7.9% (14/177) suffered severe morbidity. On multivariate analysis, only LWR <0.180 (odds ratio [OR] 2.53, 95% confidence interval [CI] 1.15-5.55) and neoadjuvant therapy (OR 2.49, 95% CI 1.16-5.24) were associated with overall morbidity. For multiple morbidities and severe morbidity, only LWR <0.180 was significantly associated on multivariate analysis with an OR of 2.92 (95% CI 1.19-7.13) and 4.62 (95% CI 1.45-14.73), respectively. Conclusions: LWR is a preoperative marker which can be conveniently applied using standard preoperative blood tests. LWR is an independent risk factor for overall morbidity, multiple morbidities, as well as severe morbidity when used with a cut-off of LWR<1.80. (C) 2020 Elsevier Inc. All rights reserved. Combined blockades of CTLA-4 and PD-1 can yield better overall complementary clinical outcomes than individual blockades, but the response rates are still relatively low. To investigate the anti-tumor effects of various combined strategies, we designed various spherical nucleotide nanoparticles (SNPs) loaded with CTLA-4 aptamer (cSNPs), PD-1 siRNA (pSNPs) or both (hybrid SNPs, or hSNPs). The results demonstrated that hSNPs could promote significantly stronger anti-tumor immune responses in a nonredundant fashion than the mixture of pSNPs and cSNPs (pSNPs & cSNPs). We reasoned that this is because all individual immune cells could receive both CTLA-4 and PD-1 blockades when they engulfed hSNPs, but it is much less likely that individual immune cells could receive both CTLA-4 and PD-1 blockades as many of them may not take both pSNPs and cSNPS from pSNPs & cSNPs. Further results revealed that the synergistic immune stimulatory effects of CTLA-4 and PD-1 blockades in the form of hSNPs were at least partly through regulating the immune suppressive function of both Tregs and TIM3(+)exhausted-like CD8 T cells and allowing effector T cells to expand. This mechanism is not identical to earlier reported mechanisms of CTLA-4 and PD-1 blockades.

Three biorthogonal click reactions, a photoinitiated thiol-yne reaction, an azide-alkyne cycloaddition, and a methyltetrazine-transcyclooctene Diels Alder, were used to independently control the presentation of several bioactive proteins to valvular interstitial cells (VICs) in hydrogel scaffolds. Exploiting the orthogonal click reactions, FGF-2, TGF-beta 1 and BMP-2 combinations were patterned into distinct regions on a hydrogel to control VIC activation and nodule formation. Cellular crosstalk between separate regions of the same scaffold was affected by the size of each region as well as the interfacial area between different regions. Collectively, these results demonstrate the versatility and robustness of a photoinitiated thiol-yne reaction to template pendant functionalities that allow for the bioconjugation of multiple proteins. This approach maintains protein bioactivity, providing an in vitro platform capable of achieving a better understanding of the complex mechanisms involved in tissue fibrosis.

Active targeting and overcoming multi-drug resistance (MDR) can be some of the important attributes of targeted therapy for metastatic breast cancer (MBC) and triple-negative breast cancer (TNBC) treatment. In this study, we constructed a hyaluronic acid (HA)-decorated mixed nanomicelles-encapsulating chemotherapeutic agent paclitaxel (PTX) and P-glycoprotein inhibitor ritonavir (RTV). HA was conjugated to poly (lactide) co-(glycolide) (PLGA) polymer by disulfide bonds (HA-ss-PLGA). HA is a natural ligand for CD44 receptors overexpressed in breast cancer cells. Disulfide bonds undergo rapid reduction in the presence of glutathione, present in breast cancer cells. The addition of RTV can inhibit the P-gp and CYP3A4-mediated metabolism of PTX, thus aiding in reversing MDR and sensitizing the cells toward PTX. An in vitro uptake and cytotoxicity study in MBC MCF-7 and TNBC MDA-MB-231 cell lines demonstrated the effective uptake of the nanomicelles and drug PTX compared to non-neoplastic breast epithelium MCF-12A cells. Interestingly, in vitro potency determination showed a reduction in mitochondrial membrane potential and reactive oxygen species in breast cancer cell lines, indicating effective apoptosis of cancer cells. Thus, stimuli-sensitive nanomicelles along with HA targeting and RTV addition can effectively serve as a chemotherapeutic drug delivery agent for MBC and TNBC.

Doxorubicin (DOX) resistance, which results in a reduced accumulation of DOX in the nucleus and hence decreased DNA damage, is a major challenge for chemotherapy against hepatocellular carcinoma. In this paper, we combined chemotherapy with photodynamic therapy (PDT) to combat DOX-resistant human hepatocellular carcinoma cells. We have prepared the polymeric micelles conjugating with DOX and zinc(II) phthalocyanine (ZnPc) through a pH-responsive hydrazone linker and a glutathione (GSH)-responsive disulfide linker, respectively. The polymeric micelles (DOX-ZnPc-micelles) exhibited a spherical shape with a size of about 98 mn diameter and showed excellent stability in aqueous solution. Due to the self-quenching of the ZnPc inside the micelles, DOX-ZnPc-micelles did not emit fluorescence upon red light irradiation. Drug release experiments verified that DOX and ZnPc could be released under acidic conditions and reducing environments, respectively. A higher concentration of DOX was internalized into DOX-resistant R-HepG2 cells through the delivery of polymeric micelles when compared with the free DOX, hence DOX-ZnPc-micelles exhibited a significant enhancement in anticancer activity. The IC50 value of DOX against R-HepG2 cells was found to be 21 mu M when combined with PDT and it was 5-fold less than that of a single treatment of DOX (102 mu M). The DOX-ZnPc-micelles could induce cell apoptosis and necrosis on R-HepG2 cells by combined therapeutic modalities, while these micelles induced only apoptosis on IiepG2 cells. We have demonstrated that utilization of polymeric micelles can significantly enhance the cellular uptake and cytotoxicity of DOX against R-HepG2 cells when compared with free DOX. Moreover, PDT can act as an adjuvant therapeutic modality and combine with chemotherapy to further improve therapeutic efficacy. Overall speaking, DOX-ZnPc-micelles can overcome DOX resistance and induce a synergistic therapeutic effect against DOX-resistant R-HepG2 cells, hence improving the therapeutic efficacy when compared with monotherapy.

A simple S-S (disulfide)-bridged dimeric Schiff base probe, L, has been designed, synthesized, and successfully characterized for the specific recognition of Al3+ and Fe2+ ions as fluorometric and colorimetric turn-on responses in a dimethylformamide (DMF)-H2O solvent mixture, respectively. The probe L and each metal ion bind through a 1:1 complex stoichiometry, and the plausible sensing mechanism is proposed based on the inhibition of the photoinduced electron transfer process (PET). The reversible chemosensor L showed high sensitivity toward Al3+ and Fe2+ ions, which was analyzed by fluorescence and UV-vis spectroscopy techniques up to nanomolar detection limits, 38.26 X 10(-9) and 17.54 x 10(-9) M, respectively. These experimental details were advocated by density functional theory (DFT) calculations. The practical utility of the chemosensor L was further demonstrated in electrochemical sensing, in vitro antimicrobial activity, molecular logic gate function, and quantification of the trace amount of Al3+ and Fe2+ ions in real water samples.
