Resistant Hypertension: Questions and Contemporary Answers

The review presents the important problem of resistant hypertension. Its real prevalence is unknown. According to population studies and meta-analyzes of clinical studies, the prevalence of this most severe form of hypertension reaches 12-15% in the general population and 15-18% in clinical cohorts. Over the past decades, an increase in its frequency among patients with hypertension has been noted. Based on the results of large-scale studies, risk factors are detected that allow to assess the risk for the resistance to antihypertensive drugs. Adherence to ongoing antihypertensive therapy is crucial to addressing the issue of hypertension resistance; there are acceptable ways to evaluate it in clinical practice. The review discusses the most common mistakes in the choice of therapy, which can cause resistance to antihypertensive treatment, namely irrational drug combinations, insufficient dosage of the drug, and the use of non-prolonged forms of drugs. The latest recommendations for the diagnosis and treatment of hypertension, including its resistant form, are analyzed. The review contains a rationale based on the results of randomized clinical trials, the choice antihypertensive strategy in this variant of arterial hypertension. The importance of fixed combination antihypertensive drugs, as well as thiazine-like diuretics and amlodipine is stressed. The results of studies demonstrate the rational for the use of antimineral corticoid drugs, namely spironolactone, for this category of patients. The authors offer an updated algorithm for the diagnosis and treatment of resistant hypertension, based on the sections of the latest clinical recommendations on this problem.

Nonlinear absorption of some thiazine, xanthene, and carbocyanine dyes

The saturated and two-photon absorption of various classes of heterocyclic dyes [thiazine (thionine), xanthene (erythrosine), and carbocyanine (3,3′-di-(gamma-sulfopropy1)-4,4′,5,5′-dibenzo-9-ethylthiacarbocyaninebetaine pyridinium salt, DEC)] possessing optical absorption in the 500-680 nm range were measured at the wavelengths of 1064 and 532 nm using nanosecond (10 ns) and picosecond (40 ps) pulses and z-scan technique. It was shown that reverse saturated absorption unlikely plays important role in those dyes. We defined the two-photon absorption coefficients of thionine and erythrosine at 1064nm using 40 ps pulses (0.8 x 10(-11) and 1.0 x 10(-11) cmW(-1) respectively). The concurrence of saturated absorption and two-photon absorption was analyzed in the case of thionine and erythrosine using various models. It was suggested that dimerization of those dyes significantly suppresses the nonlinear optical response in the case of both 10 ns and 40 ps pulses of 532 nm radiation. At the same time it was found that, in the case of 532 nm pulses, the monomers of thionine participate in the processes of saturated absorption and two-photon absorption, while J-aggregates of DEC show predominantly two-photon absorption. Physicochemical and Biological Characterization of Fucoidan from Fucus vesiculosus Purified by Dye Affinity Chromatography

A comparative study concerning the physicochemical, monomeric composition and biological characters among different fucoidan fractions is presented. Common purification techniques for fucoidan usually involve many steps. During these steps, the important structural features might be affected and consequently alter its biological activities. Three purified fractions were derived from Fucus vesiculosus water extract which, afterwards, were purified by a recently-developed dye affinity chromatography protocol. This protocol is based on dye-sulfated polysaccharide interactions. The first two fractions were obtained from crude precipitated fucoidan at different pH values of the adsorption phase: pH 1 and 6. This procedure resulted in fucoidan_1 and 6 fractions. The other, third, fraction: fucoidan_M, however, was obtained from a buffered crude extract at pH 1, eliminating the ethanol precipitation step. All of the three fractions were then further evaluated. Results revealed that fucoidan_M showed the highest sulfur content (S%), 12.11%, with the lowest average molecular weight, 48 kDa. Fucose, galactose, and uronic acid/glucose dimers were detected in all fractions, although, xylose was only detected in fucoidan_1 and 6. In a concentration of 10 mu g.mL(-1), Fucoidan_6 showed the highest heparin-like anticoagulant activity and could prolong the APTT and TT significantly to 66.03 +/- 2.93 and 75.36 +/- 1.37 s, respectively. In addition, fucoidan_M demonstrated the highest potency against HSV-1 with an IC50 of 2.41 mu g.mL(-1). The technique proved to be a candidate for fucoidan purifaction from its crude extract removing the precipitation step from common purification protocols and produced different fucoidan qualities resulted from the different incubation conditions with the immobilized thiazine toluidine blue O dye.

Crystal structure, Hirshfeld surface analysis and interaction energy, DFT and antibacterial activity studies of ethyl 2-[(2Z)-2-(2-chlorobenzylidene)-3-oxo-3,4-dihydro-2H-1,4-benzothiazin-4-yl]acetate

The title compound, C19H16ClNO3S, consists of chlorophenyl methylidene and dihydrobenzothiazine units linked to an acetate moiety, where the thiazine ring adopts a screw-boat conformation. In the crystal, two sets of weak C-HPh center dot center dot center dot ODbt (Ph = phenyl and Dbt = dihydrobenzothiazine) hydrogen bonds form layers of molecules parallel to the bc plane. The layers stack along the aaxis direction with intercalation of the ester chains. The crystal studied was a two component twin with a refined BASF of 0.34961 (5). The Hirshfeld surface analysis of the crystal structure indicates that the most important contributions to the crystal packing are from H center dot center dot center dot H (37.5%), H center dot center dot center dot C/C center dot center dot center dot H (24.6%) and H center dot center dot center dot O/O center dot center dot center dot H (16.7%) interactions. Hydrogen-bonding and van der Waals interactions are the dominant interactions in the crystal packing. Computational chemistry indicates that in the crystal, C-H-Ph center dot center dot center dot O-Dbt hydrogen bond energies are 38.3 and 30.3 kJ mol(-1). Density functional theory (DFT) optimized structures at the B3LYP/ 6-311 G(d,p) level are compared with the experimentally determined molecular structure in the solid state. The HOMO-LUMO behaviour was elucidated to determine the energy gap. Moreover, the antibacterial activity of the title compound has been evaluated against gram-positive and gram-negative bacteria.

Photodynamic activity of nanostructured fabrics grafted with xanthene and thiazine dyes against opportunistic fungi

Fungi are an important class of human pathogens for which considerable research has gone into defeating them. The photodynamic effects of rose bengal (RB), phloxine B (PB), azure A (AA), and toluidine blue O (TBO) dyes to inhibit Aspergillus fumigatus, Aspergillus niger, Trichoderma viride, Penicillium funiculosum, and Chaetomium globosum were investigated grafted to nano- and micro-structured fabrics. Three antifungal tests conducted: broth microdilution test of free dyes, zone of inhibition and quantitative antifungal assays on fabrics grafted with dyes. In the broth microdilution test, free RB displayed the lowest MIC at 32 mu M to inhibit visible hyphal growth and germination but the antifungal ability of MIC for other photosensitizers below 63 mu M was insignificant. RB and PB showed lower MIC than AA and TBO. In the inhibition zone tests, nanostructured fabrics grafted with RB and PB did not display fungal growth on the surface. Most microstructured fabrics grafted with AA and TBO showed little inhibition. In quantitative antifungal assay, nanostructured fabrics grafted with RB has the largest inhibition rate on T. viride and the lowest inhibition rate on P. funiculosum and the results showed the increasing inhibition rate in the order of AA < TBO < PB < RB.
